accessing via web GUI not working

Hello Guys,

I can't access web GUI for cisco switch ,However I can access it via SSH and telnet.

my credential didn't accepted.

Running Configuration is attached.

 - It's unclear from the screenshot what happens, is there a password-failed logon ? What happens if you try another browser ?

Nothing happens just the page repeat itself asking for the credential without any error message.

i will try another browser and tell you.

same issue on firefox and IE 

i don't know what should i do
