after upgrading 2960X switch becomes failed

everytime when I upgrade this switch the problem exposed. i checked md5 had not problem. 

 model number: c2960x-universalk9-mz.152-7.E9

where did you get this image ?

can you copy freshdownload image to USB and try to boot from USB Image ?

and post complete boot log from the time switch start to until if failed.

Roll back to the "original" IOS the switch came with and it works fine, doesn't it?

Yes my friend, I did it and the switch have worked well. I guess maybe because of TFTP I used to do because when I wanted to upgrade new ISO.bin , TFTP gave me 6 time out.

What is the current working version, is it EX3, EX4 or EX5?
