Cisco 3560x all ports go down with no led lights, yet power is still on

I have a Cisco 3560x-48T-L 12.2 (55) SE5 switch in which 1-2 times a month has an issue where all ports go dead, yet the power and fan is still running.  No lights on the front whatsoever except the SysT light.  Once I reboot it goes back to normal.  This seems to be happening a few times a month.  Now we have another switch interconnected off a port, yet that one never has any issues.  Anyone know what could be causing this strange behavior?

Your switch is probably crashing. What is the console output when the switch goes dead?
