I'm currently experiencing an issue where my 3850 is no longer booting with the IOS is goes directly to the switch: prompt.
When I do a DIR command it doesn't show the Flash: directory.
I tried to perform the Flash_Init with the Emergency-Install using usbflash0: with no luck.
It comes up with no such device for the Flash.
The USB is working I'm able to do a listing of the contents.
Is it possible that the IDE drive is corrupt?
Thanks
Wayne

here is the guide to recovery from Switch: prompt
make sure you formatted the USB right format, try use differnet vendor USB if not working.

The USB is working fine, and I went through the recovery procedure.
The problem is that I don't have any Flash: directory when I perform the DIR command at the SWITCH prompt

1. Boot from usbflash0: and after successful boot from usbflash0:
2. Format flash:
3. Copy the IOS XE what you looking to upgrade from usbflash0: to flash:,
4. change the "boot system switch all flash:version.bin.
5.reboot

Raise a TAC Case and organize for an RMA.
