cisco 4500 x series ios issue

Hello People 

i have problem in my 4500 x series switch which is in redundancy when i want to upgrade i o s for the switch after uploading i o s i have put boot command and reload . two times same procedure as it is in redundancy state but after the reload the switch is going to roman state every time even if i  reload . i have to visit the site and from roman mode i have to boot flash new i o s . i cannot get the problem so please guide me

Connect console post complete boot logs to understand the issue :

post dir flash information, show run also.

Connect the console cable and reboot the switch.  Post the entire boot-up process.
