Cisco : Switch PSU is BAD even after changing the PSU

Could be a bug. 

What firmware is the switch on?

please find below model/version details.

IOS is quite old  - Either uplift same IOS code   12.2.55-SE13  or move to 15.X

That is an ancient IOS version and I would say that it would be a bug.

not sure what you have already tried, but I would put the working power supply into the slot for the non-working power supply, and the non-working power supply to the slot for the working power supply. That means, you basically switch the power supply slots. That way, if the working power supply does not work in the other slot, it means that the problem is likely with the backplane of the switch itself.
