how to change destination address by nat ???

hii ,
i want to change destination address if the oacket matched a specifi port , is it applicable ???
as an example
any packet with "dest port of 5060 "and going to "whatver is going to " , i want to change thier destination to x.x.x.x and keep on all packet detail
how to do that by nat ??
i wish an example

ive read alot about nat , but i just want a verification from your side
assume my topology is
localnetwork>-------------
on my local network is 192.168.10.0/24
if any one from the locl network above requetsed any ip with destination port 5060 and udp port , i want to change destination to 11.12.13.14
