Get the following message with no prompts:
Authentication failed
% Authentication failed
% Authentication failed
Press RETURN to get started.
Got it to load ROMMON mode but same thing, had no prompts to login or make changes.
How do I get prompt to login to makes changes?

Ok as Reza mentioned you need to execute password recovery. Please check the following links related to Cisco 1941, Don't skip any step:

Hi,
You don't need to worry about removing the flash and inserting it back.
Just reload the router and follow the steps to break in to it.
