As I struggle with small but correlated-with-user-complaints Output Queue Drops on a 3560, I realized that even after 25+ years of working with this stuff, I still don’t fully understand exactly WHEN a Cisco switch will remove a packet from a Tx buffer after putting the packet on the wire.
In the old days of 10Base-2 when all NICs listened to shared traffic on the cable, each NIC was responsible for listening for its address, and if it thought the packet was for it, putting it into an internal buffer and sending it up the stack into the guts of the attached device. At Layer 1 and Layer 2, there was no feedback from the receiving NIC to the sending NIC.
Now we have switches with Tx buffers. Is it still the same at Layer 1?
In a switch, the packets hit the Tx buffer after going through the guts of the switch. The Tx buffers are FIFO, and eventually a packet gets a turn at the wire. So, here is the big question:
The electronics in the switch see that a packet is ready for the wire. The electrons on the Tx pairs of the cable attached to the interface are wiggled. Does the switch then immediately discard the packet from the Tx buffer and advance the next one in the buffer to the wire, or does it wait for some kind of an ACK from the device on the other end of the cable?
I ask this because I am at a loss as to what is causing the OQDs. I’m wondering if I can somehow blame the server attached to the interface. These are all Gig interfaces, there is no duplex mismatch or any errors other than occasional burst of OQDs. When the OQDs happen, the data rate on interface does not even hit 70 Mbps, (5- minute interval). The servers are fast, new Sun systems. I can’t imaging what kind of a short burst of traffic could fill the Tx buffers when the average data rates are so low.

“Applications
At times the traffic transmission characteristics of the applications used can lead to output buffer problems. NFS file transfers that come from a Gigabit attached server that uses user datagram protocol (UDP) with a 32K window size is one example of an application setting that can bring out this type of problem. If you have checked or tried the other suggestions in this document (checked speed/duplex, no physical errors on the link, all the traffic is normal valid traffic, and so on), then reducing the unit size that is sent by the application can help alleviate this problem.”
We do use NFS for NAS.

You're writing about a switch that operates at layer 2 which in Ethernet requires no acknowledgement. So long as the frame gets sent successfully, then I would imagine it would be removed from the TX buffer immediately. Reasons for it not being sent successfully could perhaps include carrier drops and collisions.
Maybe you have a dodgy Ethernet segment on your outgoing interface, I imagine you'd have mentioned if you had lots of carrier drops - but worth checking that. Or an unexpected hub somewhere? Perhaps you have a faulty NIC on the affected segment.
I'd start by changing to a proven piece of patching (including fresh patch panel ports etc) and then look at swapping out the NIC, and trying a different, proven, switch port too.
Just some ideas, hope they help.

Thanks, I appreciate the review of basics particularly the lack of ACKs for ethernet - duh. My brain is twisting ...
We have aready tried all the easy stuff like the nic, cables, moving the server to another interface, moving to another switch, etc. All of this is in a multi-million$ datacenter.

It discards it immediately. Any sort of ACK would be handled by a layer-4 protocol. IP (L3), Ethernet (L2) and Gig (L1) in this case are connectionless.
An OQD happens when the interface output queue is already full and the switch tries to enqueue a packet for transmission. The forwarding engine sees the output queue is full and opts to drop the packet. The packet isn't actually dropped from within the queue (if you are using something like WRED, it may be, but that is outside the scope of this discussion and practically won't change the solution).
As far as your actual problem, this is likely due to bursty traffic, as you seem to already have guessed in one way or another. Is QoS enabled on this box? You can probably tweak the buffers on the 3560 to alleviate the problem but ultimately it is either the cause of many-to-one oversubscription (i.e. 5 ports suddenly burst traffic out one port), or a drop in line speed (a 1000M port transmitting to a 100M port).
