I am having an interesting problem that I have never seen before. The vast majority of "even" numbered ports on a WS-X4748-UPOE+E module (slot 2) are not supplying PoE. The PoE devices are various Avaya VoIP phones. For some reason, only port 2 on that blade works for PoE, but no other even ports work. All odd numbered ports work fine. I assume that I will have to open a TAC case for this, but wondering if anyone has ever seen anything similar. There are absolutely no error messages in the logs. Here is a look at the show modules and show power inline.

What size power supply do you have?
Do you have enough power to power up everything?
What happens if you move the connections from blade 2 to blade 3 or 4?

Thanks for the quick response.
There are two redundant 4200W power supplies in the switch. There is plenty of power available. If I move connections to any other ports on the entire switch other than any even numbered port on module 2 (other than port 2), PoE works as expected. here is a show power from the switch:

Thanks again.

It is also worth mentioning that all of the ports where PoE does not work, those same ports connect to non-PoE devices without any issues, like a laptop or desktop.

Thanks for the advice. I will give that configuration a shot and post an update if it helped.

Yes, you have enough power. Can you try setting one of the ports in module 2 statically and see if the phone comes up. "sh inlin power module x" will show you how much power the working phones need. And then configure the same amount or more on one of the non-working ports
example
Config t
interface g2/12
power inline consumtion ....
If none of these help you most likley need to RMA that module.
