sfp on 3750 stack stops working

3750v2 running 12.2(55)SE7 

The sfp just goes down.  If I reseat it in the same spot it will come back up.  I have replaced the sfp and the fiber that connects it to the network.  There doesn't seem to be anything obvious in the logs.  Any ideas?

This is a common issue with 2960X switches but have not seen it in 3750s.

Try upgrading to a newer version of IOS.

do you still get the problem if you plug the SFP into a different port?, does the SFP MM or SM match the fibre type?

Yes it is still a problem in a different port.  Even in a different switch on that stack.  I haven't checked the mode of the sfp.  I changed it out but didn't pay attention.  I will check.
