looks like a power supply issue.. the power supply is currently in OFF state.
could you check if the power cable is working fine? if possible replace the power cable.
is there any other impact due to this error message?(like all the other moduels adn the PoE devices are working fine?)

I have already:
1.turn off/on;
2. fisically detach power suply;
3.change cable.
There are no other impacts.

Kindly run the following command:
Switch#show environment st

power-supply 1 power-output-fail: failed >>Please check if you are seeing this error?if yes then replace the PSU.
Switch#sh environment alarm
environmental alarms:
system minor alarm on power-supply 1 power-output-fail (raised 2w0d ago) >>check this msg as well.
