Switch Management Login Fail

Hello all,

I have received a SG250X to configure. However, after resetting the device to factory defaults, I cannot seem to login to the switch management application. It does not give me an error or "incorrect username/password"; it simply goes back to the login screen.

I can login to the SNA with the default "cisco" username and password but not the GUI to configure the switch. The switch is also refusing to connect via SSH or telnet as well. Any and all help is greatly appreciated.
