This message typically means a hardware failure.

If this switch is under contract, it should be RMA'd. No further troubleshooting to be done. ( you may see tht some suggest to reload and format the flash and upload the new ios, but ths will not resolve most of the time. Hence avoid the delay and order the new switch by creating the RMA.)
