WS-4748 ports not granting POE power

4510 switch with 4 WS-4748-UPOE cards and now 2 of those cards are not granting POE to Cisco 8851 phones. Plenty of power and these cards were granting power and have some phones on them. The issues just seems to be on cards 1 & 4. Cards 2 & 3 will grant power.

Indeed a quick fix is to reload the module or the whole switch. Issue symptoms are observed as you said new PoE won't be granted neither PoE changes will take effect. btw you are hitting CSCuz38242/CSCum48844.

Possibly a software bug that is causing this.  Take a look at the release notes for the version of the IOS you are running. You should also contact TAC for this.

I'm agree with Reza: What IOS version is the chassis running on? I too suspect an IOS bug.
So the port won't power 8851? Only an 8851? How about other model of phones?

Typical PoE issues mostly are resolved with a reload unless the asic is bad then the switch needs to be replaced. Check the logs for poe messages to identify related bugs.

I rebooted the switch after hours last night and the problem showed up again. But this time these two error messages immediately showed up in the log. I did not see these before as they may have occurred weeks or months past as the switch had been up over a year and only the past several days did we move phones around.
*Oct 11 03:34:56.193: %C4K_ETHPOE-4-SUPPLYFAULT: Module 1 POE controller has been shutdown because of excessive power usage.
*Oct 11 03:35:01.086: %C4K_ETHPOE-4-SUPPLYFAULT: Module 4 POE controller has been shutdown because of excessive power usage.
This morning I did a "hw-module slot 1 reset" on each card and that seems to have fixed the issue. Ports that were not providing POE are now working.
So it appears that if a POE device is connected when the above error occurs they continue to work but if you move a device or connect a new device they will not till a hw-module slot x reset is done.
