I' ve a cisco WS-C6509 with a SUP-1A and a MSCF2 and a PFC.A Catalyst running Catos 5.2.1 version via xmodem download, but the system crashed after It finished to complete the System Power On Diagnostics.
That definitely appears to be software issue. Verify that you have loaded the correct software to the supervisor and it meets the minimum RAM requirements. If it does I would try to use a more recent version of catOS and see if that fixes the problem. If that doesn't work I would recommend putting in a TAC case for further assistance.
