Newly installed IX5200 crashed during calls and does not function anymore....

The system was originally shipped with 8.0.0, we have upgraded it to 8.3 then 9.0.0(19), everything was fine for a week and a half. Suddenly this morning after 5 to 10 mins during the call, all screen went black and Touch 10 shows lost connection. Tried a few times reboot and the touch 10 got back online but the system was kinda broken, web page will automatically log out showing a json page: {"error" : "incorrect_client_credentials"}

 

We tried factory reset few times both on partition 1(9.0.0) and partition 2(8.3.0), nothing changed. And this error stops us from completing the first time setup. 

 

the host cpu and encoder were diagnostic checked, all items pass.

 

log attached, any help?

 

PS: filed a DOA, but would like to know what is going on in the system.
