Application User Password issue on Version 11.5.1

Hi I was wondering if anyone else has seen the same issue. 

I am unable to log into the CCM admin page on the publisher with my credentials, as a full CCM administrator, returned with invalid user ID/Password.

I then SSH to the publisher  carry out a utils reset_ui_administrator_password entering the same password which returns a fail, I guess as the password is the same. I then change the password and then reset it back to what it should be and then I can log in ok.  THis has now happened twice on the same cluster.

The version is 11.5.1.11900-26

Any feedback to is welcome even if it for my own sanity :-)  - in over 10 years of working with CCM I have never seen this before and it is only happening on one cluster we support.

Command to change Application password is

utils reset_application_ui_administrator_password
