“dropping to debug shell sh can't access tty”. while upgrading firmware for M220 M4S

Hi

I have cisco new c-series server (M220 M4S) and while upgrading firmware from 3.0(3a) to 3.0(3c),I m getting below error message

“dropping to debug shell sh can't access tty”.

So please can some help what would be cause and to fix the issue.

After mounting ISO Image(ucs-c220m4-huu-3.0.3c.iso) and booted from ISO image ,then Cisco Host utility Upgrade loads the files ,but while loading files, I get above errors.

Note: Same ISO image I used for another server with same model since we have two brand new servers  and I can successfully upgraded the firmware for one server
