Help upgrading licenses UCNX from 9.x to 11.x

I'm having problems migrating licenses from Unity Connection 9.x to 11.x. The migration is on new hardware. I have a PAK with the units of basic messaging to migrate.

I enter the PAK on license portal, then I enter the license request from ELM 11.5 but after that I can't finish the process. The button to finish the process (next) is greyed out.

I'm not sure if I'm following the correct process.

For what its worth i had this happen to me last week and had the same experience using different browsers and even different computers.  Ended up having to contact licensing and have them generate the license for me.
