PCA alert for call failures

I am hoping that I am simply missing this but we just experienced an issue with our SBC that caused about 600 calls to fail. this is about 50% of our total call volume and Prime Collab didn't make a peep. I am running version 11 but is there a threshold setting for any alert that will be triggered by the failure of calls? I see the failures in the service experience tab but no corresponding alert.

What was the failure? More importantly, did anything in the call path throw an error? For example if CUBE sent a normal BYE to CUCM with a cause code of 16 (normal call clearing), then how would PCA know that isn't just half of the calls in the environment hanging up normally (eg a large conference call)?
