"Panic Button" intercom issue

We have a Cisco Phone System running Communications Manager and Unity 11.5.1.13902-2.  Originally we setup a "Panic Button" type setup where a soft button is configured on every phone.  If you push this button, it automatically dials our security office, automatically puts the security office side on mute, and the security people can see where it called from with the full description and they get to listen in on a potentially dangerous situation. In addition to this, we have the panic calls forwarded to a Cisco portable phone so that they ring on both the desk phone, and the portable. 

 This worked for a while, but a few months ago, we found that panic calls pickup and automatically mute as desired, but they display the location/call information  of the originator for just a second or 2 at the most and then the screen goes blank.  This obviously is extremely bad as our security officers have no way of knowing who they are listening to.  The system previously showed the extension and location information for the duration of the call.  We tried updating Communications Manager, Unity, and the attendant console to the latest versions of 11.x but this seemed to make no difference, even with the respective firmware updates to the phones. 

Any information or insight on how to fix this issue would be greatly appreciated.  Our security department is very nervous not being able to see where a "panic" call is coming from and I would to get this functionality back ASAP.
