Unable to add Unity Subscribers

We are unable to add unity subscribers because the “Add Subscribers” dialog’s Exchange Information pull downs are empty (see attachment screen shot). In addition none of the buttons on the dialog accomplish anything (i.e. clicking the cancel button does not close the dialog).

This had worked fine up until about a week ago when we added a new Exchange server to our site.

Unity 3.1 Build 3.1(3) Running on Exchange 2000 Standard server (SP3) on Windows 2000 Server (SP4).

Anyone have suggestions for how to resolve this issue?

Does the cisco import tool work?

Are there any error messages in the event viewer logs?

Using the import utility I'm able to create accounts okay. There are also no errors generated in the event logs.
