Hi,
We are planning to migrate the existing licenses and Data from CCM 4.1 to a New MCS Server with CUCM 8.5 .
The old server is end of life and no valid SMARTNET.
1.What are the items to be ordered ?
2. What is the procedure for migrating data & license to the new server ?

You don't specify whether they have UCSS or not. If they don't have UCSS use either 3x3 or CUWL for migration

You will need to order an upgrade Kit for CUCM 8x, here you can find the ordering guide:

Now regarding the upgrade process, you will need to use the DMA (data migration assistant), for CUCM 7.1.3 since there is no direct upgrade from CCM 4x to CUCM 8x
The DMA will copy the DB info and reorganize it on the new DB format and give you a tar file and a file that you will need to upload to cisco.com in order to get a licence file for the current devices that you have on CCM 4x

Once you have this, you will need to build your new 7.1.3 server and select the option 'upgrading from windows'
This will ask you for the tar file (DMA file) you can use an FTP to point the installer to the file.
Once the CUCM 7.1.3 is installed you can upgrade to 8x using the files on cisco.com or the DVD from the upgrade Kit.

On the license side there is a form for automatically generating the 7.x license (which you need to do before migrating to 8.x):
Scroll down and click on:
Register for Upgrade/Migrate License
CUCM is an option. The DMA tool on Windows will prestage your license file (make sure the mac address is right) otherwise you can also get it from the 7.x publisher once installed and DMA run as part of the install wizard.
You will need to go through the normal license process for registering the PAK code from the CUCM 8.5 media you get sent so that you have the 8.x feature license for CUCM.
I did a similar migration about 2 months ago from 4.2(3b) -> 8.5(1)SU1 and it went pretty smoothly. There is a migration guide http://www.cisco.com/en/US/customer/docs/voice_ip_comm/cucm/upgrade/7_1_2/upgrd712.html from 4.x to 7.1(2) that covers a lot of good points of the overall process.
