Check the following CUCM upgrade tool. Just put in your current and destination versions and it will let you know whether direct upgrade is possible or step upgrade is the only way. It will help you out with licensing part as well. Explore the link

Hope that helps

Yes, you need to be on 6.X or 7.X to upgrade to 8.X, there's no direct upgrade from 5.X
No, there's no link that will give you all the steps from 5.X to 8.X.
The steps are:
a) Choose whether you're upgrading to 6.X or 7.X as the middle step and follow the upgrade link for the version.
b) Once you're on 6.X/7.X follow the upgrade guide for 8.X
If you've done any upgrade before it's exactly the same thing.
The process would be just slightly different if you're going to UCS, but there's a document that covers upgrading to 8.X on UCS where all the other install and upgrade guides are.
DLUs and Node licenses will be migrated with the upgrade, but you'll need to get a SW feature license for the 8.X version you're going to.
