User has problems when pressing the # prompt

I have one user out of 450 that when he tries to reply to a message he gets dead air after pressing the # sign. It is an intermittent problem and happens to him from several different phones and at different locations. He has no problems using the # sign to login to his profile but after he leaves his reply message to the user that had called him and presses the # sign to send he gets dead air. We are release 4.0(3)

I kinda doubt this has anything to do with the # key - I suspect you'd get the same thing just timing out and having the record terminate - are there any errors in the event log when this happens? Seems almost like there's an issue punching the stream file into the user's mailbox and sending it on behalf of them - there should be errors in the logs if that's the case...

I'm seeing TSP warnings. We are 4.0(2a) CM and 4.0(3) unity. The unity TSP is 7.0(3). Is it possible that the TSP is causing this problem?
