Just completed a new install of Unity UM 4.0.5 on a 2003 SP1 server with exchange 2003 SP2.
I have only setup the first user but when we try and playback a message in the users Outlook we are prompted to login.
The resourse is the Unity server and we are prompted for username, password and domain even though the user is logged into the domain corretly. If we enter the users credentials we are unable to playback but if we use the UnityInstall account the phone rings and we can listen to the message.
Any ideas?

Run Cisco Unity Permissions Wizard version 2.1.0.32 or later. This version of the Permissions Wizard is shipped with Cisco Unity 4.1 and later.

That has solved the issue, re-running permissions wizard with the later version has worked.
