CallManager Unity Integrated Mailbox?

We have CM 4.1(2) running in the lab and want to configure the integrated mailbox feature. The CCO documentation on how to do this is cryptic at best.

We have the voicemailbox.asp file from our UM 4.04 server, where exactly on the CM server do we copy it to?

Also, where do we configure the CM / Unity mailbox administrator credentials?

Did you ever get this working? I followed the instructions (which are extremely lacking and cryptic, as you said) to the letter, but cannot get the link to show up on my CCM user admin pages.
