faulty slot - can't boot from ISO?

I have a rather odd problem. In one of my UCS environments I seem to have one particular slot in one chassis that is broken. Nothing in that slot can boot from ISO. Some environment details:

FI's: 6248, 2.2(5b)

Blades: B230 M2

Steps taken: Decom-reack blade, swap blade, delete and recreate service profile, connect to UCS-M from different client (Windows and OS X)

Symptoms: Any blade in that particular slot refuses to boot to ISO. Other identical blades in the environment have booted from ISO, no problem. 

I'm at the end of my experience and knowledge here. What can I do to determine the cause and fix it. Is it possible that this particular slot is bad somehow? I have ruled out that it was a problem with a particular blade, Java client, or service profile. The only thing that remains the same is the physical slot location.

We will need to take a look at the chassis and UCSM logs for this.

Please open a tac case for this, and generate Chassis (with IOM suboption), and the UCSM tech support, attaching to the case.
