Cisco TAC reproduced the problem; apparently Leonardo's getting a worse result with no ports displaying at all. So we know this is a problem with Cisco Network Assistant.

Ended up opening a ticket with Cisco TAC. So far, no progress, though one suggestion to try CNA 6.2 allowed an older 4507R switch to become visible.

I ended up with this problem after migrating from an old 4507 chassis as my core L3 switch to a pair of 4500-X L3 switches. Windows just would not sync with the 4500-Xs and the switches would call my PDC insane as well.
Ended up syncing the PDC again...
