Is it possible to have WPA Enterprise + username/password + client certificate?

I want to be able to authenticate the user as well as the user's machine so that only devices that I've provisioned could get on our wifi.  Is this type of setup generally possible?

It's not how Microsoft devices work. Windows typically authenticate machine or user not both. Take a look at this link.

I ended up just doing Computer account authentication with certificate.

Exactly Scott, this is not the way authentication works in windows..., you can either try username-password  or you can authenticate via the certificates....
