Unable to import more than 1 site from CSV file into CMX Presence

I have CMX Presence 10.5.0.206 running on a 3365 High-End and I have created a CSV file as specified by CMX to import all 500 sites. When I import and click Finish Import all the sites go away. I tied also just doing a CSV file with 100 sites and still no successful import.

 

I can only import 1 site successfully from a CSV file.
