I have tried twice now to upgrade a CRS 3.5(3) server to version 4.0(2). Here are my install notes:

3/1/2006 First Attempt
Drop disk
Accept Prompt to install Windows installer may require a reboot
Installer fails continue with installation
Click OK for Cisco Security Agent message
Accept LDAP information > Next
Accept LDAP cluster profile > Next
Click OK to create profile if prompted
Windows user creation: CRSAdministrator / xxxxxxxxxxx> Next
Type in the address of the Publisher
Insert the Administrator password and private phrase > Next
In the Upgrade CRS 3.5 window > Click Backup
CRS pre-backup/restore tool returned error. Setup will now abort. > Click OK
3/2/2006 2nd attempt
8:48AM drop disk
Prompt for Window Installer 3.0 > Click OK
Instimsi30.exe failed to initialize > Click OK
Click Yes to continue installation
Click OK in the Cisco Security Agent window (Verify that Virus scanning software is disabled)
Click Next
Accept LDAP information > Next
Accept LDAP cluster profile (acdtest) > Next
Click OK to create profile if prompted
Found CRS 3.5 > Click Next
Windows user creation: CRSAdministrator / xxxxxxxxxxx > Next
Type in the address of the Publisher
Insert the Administrator password and private phrase > Next
In the Upgrade CRS 3.5 window > Click Backup
CRS pre-backup/restore tool returned error. Setup will now abort. > Click OK
Is there an installer log that I can review to try to isolate the problem. I am in the process now of upgrading all my MCS BARS versions and running BARS against my CRS application server and my CRS Historical Server. Anyone else experience this? Thanks in advance.

Check out this thread from a few weeks ago. Alex from Cisco identified a problem that sounds very much like what you are running into.
Cisco Field Alert
Effective Date: February 3, 2006
Affected Products: Cisco IPCC Express and IP IVR
Affected Releases: Upgrades from any CRS 3.x to CRS 4.0(1), 4.0(2), and 4.0(3)
Issue: While most customers have successfully upgraded to CRS 4.0(x), issues have been found in some upgrades from IPCC Express or IP IVR 3.x to IPCC Express or IP IVR 4.0(x). This issue does NOT affect new installations for 4.0 or add-ons to existing 4.0 installations.
Impact to orders in progress: Effective immediately NEW PRODUCT HOLD is being placed on all upgrade product IDs, both for purchase and via SASU, for the affected products. This issue does NOT affect new installations for 4.0 or add-ons to existing 4.0 installations.
Resolution: Resolved in the CRS 4.0(4) release, with a target release date of mid-April 2006.
Recommendations: If your IPCC Express Edition or IP IVR 3.X customer has already ordered or is planning to order an upgrade to 4.0, please inform them that we recommend they wait for the FCS of the 4.0.4 release.
