Hi Yvonne,
Yes, this is the way to aproach this. Have a look (includes part #'s);
AVVID CD Software Request Cases FAQ
Q. What do I need to do if I have lost my CDs or need another set?
A. Refer the customer to the Product Upgrade Tool
The customer is required to enter their Cisco Software Application Support Plus Upgrades (SASU) contract number and request the CD/CD set.
From this excellent doc;
http://www.cisco.com/en/US/products/sw/voicesw/ps556/products_qanda_item09186a00802262ed.shtml
Hope this helps!

thanks, Rob
I already made a request through PUT, but during the process, there is not a detail list of what CDs are included. do you know if the Operation System CD is included in that package? thank you
