Music On Hold not refreshing?

I'm getting callers saying they hear older tracks from the music on hold. I have deleted everything file that was showing in File Management and Audio Source. Callers still here it while waiting in a Queue - but it seems like it happens randomly, not consistently - other times the new file plays.

Did you upload the file to each call manager? They don't replicate, so you have to do that on each server in the cluster.

I did upload to both. I have been able to narrow it down a little. The caller only hears the old music when agents are all busy.
