I have remote agents using IPC who (occasionally) receive "Request Operation Failed" from CAD when trying to transfer a call. What causes this?

Hi Chris,
I believe you have a Blind Transfer button on CAD configured at the workflow level that enables the transfer?
If so, then you would be hitting the following defect:

Basically we have documented the actual information on the Blind Transfer on CAD in the CDA guide and what would happen during a failure.
