Does Cisco Unified Workforce Optimization Quality Management 11.5 support CUCM 12.0?

According to documentation for SR7, it does not.
Quality Management supports the following environments:
• Cisco Unified Contact Center Express (Unified CCX) 9.0, 10.0, 10.5, 10.6, 11.0, 11.5 and 11.6
• Cisco Unified Communications Manager (Unified CM) 8.0, 8.5, 8.6, 9.1(2), 10.0, 10.5 SU2a, 11.0(1a), and 11.5
• Cisco MediaSense 9.1, 10.0, 10.5, 11.0, and 11.5

The latest ES of Calabrio QM supports UCM 12.0
To get the latest version ES you will need to open a Cisco TAC case and they will pass you over to Calabrio Support who will give you access to their ftp site so you can download the latest ES.
Calabrio ES's are full install versions so you don't need to install the SR and then the ES, just install the ES

So do i need any smartnet contract for such a TAC case or it can be opened for free. I just want to test it firstly.

I don't believe you can open a TAC case without a contract and the CCX PDI has closed so you can't go that way.
