Hi,
Here are your answers:
1. Cisco call manager is an appliance box used for call control for an enteprise HQ network. Call manager Express is a Software feature which is used on Cisco ISR routers for branch office. When you have Cisco call manager express (CME) feature on your router, the router acts as a call control agent and your IP phones register to the router itself.
2. They both are same. The termanology has been changed from Call manager to Cisco unified call manager.
3. Cisco Unified Call manager express is the same as described in the point 1.
