Call Transfer from SIP Phone getting disconnected

When we transfer the calls from SIP Phones call is disconnected

When a incoming call hits my 7821 SIP Phone while transferring the same to another SIP Phone call fails 

It is working fine with SCCP Phones

Do You use Call Manager or Call Manager Express, and which version?

If CME please try to add command in telephony-service

Also U should upgrade firmware on IP phone
