CCME: Changing Calling Number on CFwdALL

I am trying to get my 2811 base CCME to work fully with a new SIP trunk provider. 

What is failing is being able to forward all calls say from a particular extension to

someone's cellphone.  e.g. user with 7965G phone hits the CFwdAll button and

enters 9212555666.  When someone calls that user's desk phone they get a

"we're sorry your call can not be completed as dialed".  As soon as CFwdAll

is turned off - calls come into the desk phone ok.

The problem according to the SIP provider is that the "calling number" in the forwarded

call leg is that of the person originating the call to the 7965G desk phone.  The SIP

provider only accepts calls with a "from" number as one of their own provided DIDs.

So what I want is when calls get forwarded - the "calling number" changes to that of

the 10 digit phone number of the desk phone being called. 

I tried enabling alternately h.450.3 and h.450.12 per the document below.  But still no

luck.  Anyone have a magic recipe?  The SIP trunk provider is 8x8 (aka Packet 8).
