CCME dialing plan / patterns for US PSTN

Hi Folks,

i´m currently configuring a CCME router with a T1 connection (TWTelcom) in Florida.

Could anyone help me out with a generic dialing plan / patterns for the US or send a link to some more informations?

I would like to understand how the PSTN is designed with regards to long distance and local area prefixes etc. and
what else should be integrated into a dialing plan (like 911 or other special numbers).
