I dont understand the difference between POTS digit manipulation in CME and Route pattern for CUCM

At some level they're the same, you use them to route calls outbound, the main difference that comes in mind is that a POTS DP will discard exact matches while route patterns won't, and there's a different way to discard digits.
RPs can also add/remove digits in the same page/config, while DPs require translation-rules or some other configuration in the IOS.

I think Java explained it very well, I will add a note taking it wayyyyy basic:
- POTS dial peers are part of IOS and can be used to route calls for CME/CUCM to specific devices, like a PRI or a fax machine directly attached to the router.
-Route patterns on the other hand can route directly to a device (as long as it's not part of a route list) like a gateway or a trunk, or, they can route through a route list which can then serve multiple devices in a route group.
