Since you said other phones do hear a Spanish prompt with the same configuration, means the Spanish Locale file is already installed.
now pls check in the problematic phone's configuration page, the 'User Locale' and 'Network Locale' should have the Spanish Locale file selected.

Hi,
beyond what Venkatesh already said ... double check if the not working Phones (phones that the announcement are not in Spanish) are registered in a Subscriber that you didn't install the 'Spanish locale'.
