Hi,
I have 3 Cisco 7912 phones with the same problem. All 3 phones have the red and green light on constantly and with a blank screen.
I cannot reset the phone since it doesnt boot properly. I tried the various suggestions in this forum regarding connecting the phone using a crossover cable and a TFTP server. I have the cp7912r.zup file but the phone is not requesting any such file to be loaded. On connecting the phone to my PC, I only see the following messages in the log viewer of the TFTP server:
Rcvd DHCP inform Msg for IP 150.16.71.113, Mac 10:78:D2:F7:F4:43 [21/06 09:31:55.928]
Rcvd DHCP Discover Msg for IP 0.0.0.0, Mac 00:15:62:59:A8:E4 [21/06 09:31:56.941]

DHCP: proposed address 192.168.1.2 [21/06 09:31:56.942]
3368 Request 2 not processed [21/06 09:31:56.945]

Any suggestions on how to fix this problem?

You need to set dhcp option 150 and/or 66 in tftpd32 for the phone to accept the address and go to the following step.

I already tried these options. Neither 66 nor 150 changes anything.. Continue to see the above messages

Some versions of tftpd32 need a special syntax, or an hex coding to understand that these are IP options Vs string options.
You can search on the matter.

Thanks for the hint. I am using the tftpd software from http://tftpd32.jounin.net/ and so far have not found anything regarding the syntax.
But will keep searching...

Thanks, I changed the code 66 to hexadecimal 0x42 in the .ini file but nothing changes. Also 0x96 (150) but in vain..

I'm not sure about your configuration but I can tell you that once the options are set correctly the phones will load firmware.
You can also use a cisco router to set dhcp correctly.
