if its a 191, you are effected with the BUG ID CSCvp92985. Fix this Bug first.

Device is connected to voice Vlan and only can be seen in Call Manager!
But it is working as analog line!
Web enabled but refuses to connect!

If this is a SPA122, it has two ports. Internet and Ethernet. HTTP traffic on the Internet port is disabled by default, but if you connect your pc/laptop to the Ethernet port, the SPA122 should assign you and IP address on the 192. range.
I don't have much experience with SPA122, but on the ATA192 that's the case. When connecting to the Ethernet port, you will get an IP address on 192.168.15.X by default, and the ATA IP is usually 192.168.15.1.

From Ssh message, its shows model as SPA122. You can find the model number from the device itself and If its ATA 191, you are effected with the BUG ID CSCvp92985.

Second thing, what's the status on CUCM for this device ? is it shows registered ?

To upgrade it though CUCM use cmterm-ata191.12-0-1-0301-002.k3.cop.sgn file. From CUCM os admin page. Install the COP on all node. Change the default ata load to the new and restart the ATA.

firmware loaded on ata 191
now I can see in Wireshark
Software version: ATA191.12-0-1-0301-002.loads
But there is problem now!
Device is not registering at all and
restarts every one minute by itself!
I can see that when connected device remembers the ip address from the previous!
