At the end of the boot process I see the following message on the screen. (See the attachment) The phone registers with Cisco Unified Call Manager works fine. I'm concern with this error message. If the phone is not communicating with the tftp server how could the phone get its configuration?

Have you recently tried to upgrade to this firmware version? If so, when you installed it did you restart TFTP, and did you install it on your TFTP servers as well as your publisher?
DOes it affect all phones?
Basically the phone will look for SCCP45.9-4-2SR1-1S.loads on your TFTP server. Check you can find this yourself and download it with a TFTP client.

In addition to the good points suggested by Aaron [+5] , apart from checking if the SCCP45.9-4-2SR1-1S load is installed on the TFTP server and TFTP service has been restarted, do check if this load is mentioned in the defaults loads under Device > Device Settings > Device Defaults as well as under 'Phone Load Name' setting on the IP phone configuration page on cucm. Also check what is the actual load on the IP phone under Model Information > Load Name.
