Is phone getting proper option 150 to point to CUCM TFTP IP address from DHCP? When you changed it to disable DHCP did you hard code this TFTP server on the phone?
What does the phone show in the status messages?

DHCP is Disabale, its not configured our CM ,
we are given the static configuration of all the ip phone
DHCP already setup the network team it is perfect ,
Remaining all the phone are working fine, only for one phone got problem
Phone status "unknown"

You should not run DHCP on CUCM server, you run it on your centrelized or local DHCP server/router.
So, if you are using static IP addresses on the phone did you verify that you have set the TFTP server on the phone to point to CUCM server?
What does the phone show in the status messages on the phone?

Is there any sort of firewall which may be blocking TFTP traffic between the Phone and the TFTP server?
Are you able to ping/traceroute from the Router onsite to the TFTP server using the LAN as a source?
Failing that, try a factory reset on the phone and input the Static IP details and TFTP server addresses again.

I can't help you find out if there is a Firewall, if you are implementing this as part of the IT team then you should be able to find out on your own if there is a firewall between the phone and the CUCM server, its not something I could know. If its for a bank then its likely there is a firewall.
Is the remote location connected via a VPN?
Does the SIP phone have a directory number assigned to it? I believe its needs to otherwise it won't register.
