Conferenced calls failing

I have a quick question regarding ad-hoc conference call issues in UCM 8.6.2 environment. Recently I have noticed a few instances of ad-conference calls failing while trying to add 3rd party in the call. I want to know where I should check to find out the root causes of such problems. If it is in RTMT, can someone tell me where exactly in RTMT I should be looking at and what I should be looking for?

Thank you very much. I will now either go ahead with changing the codecs to g711 or configure hw cfb in the gateways.
