I make test calls with the phone plugged into the port, but there is no dial tone.
You call the port it rings and answers, but when you connect a fax it is getting a dial tone error.
The other 3 ports are not configured but they can make outbound faxes and have a dial tone, but they cannot except inbound faxes.
The port I configured can accept inbound faxes, but cannot make outbound faxes, since there is not dial tone, the fax machine gets the dial tone error.
Is there a command that I am missing:
Do I need to apply a outbound dial peer? (If inbound and outbound are working on a bud set, I should not have to.)
Is there a command to turn on the dial tone that I am missing.
I originally put groundstart on the port then changed it to loopstart, could that be the issue?

Few questions:
1. Is it on CME or CUCM Voice Gateway?
2. Could you please post the full config of the router (please attach in a notepad)

Is the handoff from your telco an FXO port?
Also it's a BUTT set not a BUD set..In this case BUTT = Basic Universal Telephone Test set.
