Try updating IOS. If still trouble, you can use a voice translation-rule or SIP profile to add it back.

What IOS version are you running, I've seen similar behavior on 15.1(3)T. I've seen this in one direction only, from cucm to itsp. In the other direction, itsp to , the gateway sends the full +e164 number (which is what I want).
Outbound
Dial-peer voice 1 VoIP
Destination pattern .T
!
Inbound
Dial-peer voice 2 VoIP
Destination-pattern +3130.......
!
When adding a second outbound dial-peer that starts with destination pattern +.T the + is send!

Yes, the problem is inbound and already using latest IOS for this old box 15.1.3T4. Seems like a bug, right? I manage to add it back via a trans rule and not a sip profile but anyway I want to know if this is normal behavior or maybe o config issue or just a bug.
